Naftalan has long been used in the rehabilitation of patients with musculoskeletal disorders. Considering the complex chemical composition of naphthalene oil, many researchers rightly believe that a new direction in naphthalene therapy should be developed by the production of a large range of naphthalene preparations, and, if possible, the use of naphthalene baths only where it is strictly indicated. Prospects for the development of naphthalan therapy are related to the further search for complex, highly effective effects in joint diseases.
For the first time, we obtained and successfully used naphthalene-dimexide emulsion (NDE) in patients with rheumatoid arthritis (RA) and osteoarthritis (OA). The emulsion has a more pronounced therapeutic effect than naphthalan and dimexide alone.
In the literature, there is a lot of data on the ineffective use of one natural factor in the form of monotherapy in the treatment. Osteoarthritis is a dystrophic disease based on the progressive degeneration of articular cartilage, up to its loss, accompanied by reparative growths of the marginal surfaces of articular bones and secondary synovitis. Therefore, to increase the therapeutic effect, it is advisable to combine or combine physical factors.
For this purpose, we used NDE in combination with a magnetic field (PEMP) in patients with OA. The study was conducted in 45 patients with OA. Polyosteoarthrosis occurred in 10 patients, gonarthrosis in 30 patients, coxarthrosis in 5 patients. Secondary reactive synovitis of knee joints was observed in 4 patients. According to the method of treatment, patients were divided into II groups. According to the clinical and functional characteristics of the patients, the groups were comparable. I-gr. - 30 patients were smeared with naphthalene-dimexide emulsion on the area of the affected joints, then this area was affected by a magnetic field, the device "POLYUS-1" - P - a similar inductor, transversely, in constant mode, sinusoidal current form, magnetic intensity 25 mT, 1 minutes, No. 10 procedures
(NDE + PEMP).
II group - 15 patients received lubrication of the joint area with tar-free naphthalene, followed by a magnetic field effect on this area, according to the above-mentioned method, for 15 minutes, No. 10 procedures (ON+PeMP).
All patients were prescribed massage and physical therapy. The following research methods were used: patient quality of life (QL) questionnaire, intensity of pain syndrome (according to the VASH scale), the degree of analgesic effect of the medical factor being studied (according to the COD formula); x-ray examination of joints. Among the biochemical indicators, malondialdehyde (MDA), catalase and glutathione activity, blood viscosity, hematocrit, and the degree of erythrocyte aggregation were measured. From the immunological indicators, circulating immune complexes (CIC), anti-cartilage antibodies (AKA) and the nitro blue tetrazolium (NTT) test were determined.
During the analysis of the results of the therapy carried out in group I, out of 16 points of quality of life indicators, 10 showed a significant improvement (Р<0.01), a decrease in the quality of life index (Р<0.001) and pain intensity (Р<0.05) was also observed. the degree of analgesic effect of the studied therapeutic factor was assessed as average.
Signs of synovitis in 2 patients were not determined after treatment.
Biochemical and immunological indicators in this group of patients underwent significant (Р<0.01) changes for the better.
On the basis of clinical and functional indicators in patients with OA who received NDE+PeMP, improvement was determined in 76.7% of patients, without effect in 23.3%.
In the II group of patients who received OH+PeMP lubrication after treatment, 6 of the 16 points of determining quality of life showed a significant improvement (Р<0.05).
The quality of life index, pain intensity in this group of patients underwent changes in the smaller direction (Р<0.05), the degree of analgesic effect of the treatment factor was estimated as average. Signs of synovitis in 2 out of 3 patients were not determined after treatment.
From biochemical studies, against the background of a significant increase in catalase (Р<0.05), a tendency to increase the antioxidant activity of blood, glutathione and a decrease in malondialdehyde (Р<0.05) was observed. This improves the rheological properties of blood (Р<0.05), and even positive dynamics of immunological indicators.
On the basis of clinical and functional indicators in OA patients who received VIN+PeMP, improvement was determined in 60% of patients, without effect in 40%.
The therapy (NDE+PeMP; ОН+PeMP) has a beneficial effect on the patient's quality of life, POL, rheological properties of blood, and immunological indicators. The combined use of naphthalene-dimexide emulsion with a magnetic field gives significant and more effective results than detarsified naphthalene.
